Form PHP must call the procedure 'check reader', on the form, 4 text fields:
Name, address, date of birth, phone numbers - separated by commas.

Persist all to be in the 2 tables:
1: reader (ID, fullName, address, birthday)
2: readerPhones (number, reader_ID)

First you need to send your name, address, and date of birth in the table, the `reader`.
Then get their id, parse the numbers, and send them to the table readerPhones.

Can't understand how it all crank. How to send numbers, a string with a comma in another form? As in steam rooms where, if SQL does not have arrays. In General, if someone is not difficult, throw example.
